This Privacy Policy explains how Summit Health Clinic ("Summit Health," "we," "us") collects, uses and shares information when you visit weightloss.summithealthmo.com (the "Site") or submit an appointment request through it.
Please note: the appointment request form on this Site is a general contact form, not a patient portal. It is not a secure or HIPAA-compliant channel. Please do not submit medical records, diagnoses, test results or other sensitive health information through it. Information you send us before you become a patient is generally not protected health information under HIPAA.
1. Information we collect
Information you give us. When you complete the appointment request form we collect your name, phone number, email address, preferred contact method, the general reason for your inquiry, and anything you choose to write in the message field.
Information collected automatically. Like most websites, we and our analytics and advertising providers automatically collect technical information such as your IP address, browser and device type, referring page, the pages you view, and how you interact with the Site. This is collected through cookies and similar technologies.
2. How we use information
- To respond to your request and contact you about appointment availability
- To schedule, provide and administer clinical services if you become a patient
- To operate, secure and improve the Site
- To measure the performance of our advertising and understand how visitors find us
- To comply with legal, regulatory and professional obligations
3. Advertising and analytics cookies
We use Google Analytics and Google Ads conversion measurement on this Site. These services use cookies and similar identifiers to help us understand how visitors reach and use the Site and whether our advertising is effective. They may collect a device identifier, IP address and details of your visit.
We do not use this Site to build advertising audiences based on health conditions, and we do not knowingly share information indicating that you are seeking a specific medical treatment with advertising partners for their own marketing purposes.
You can opt out of Google Analytics using the Google Analytics Opt-out Browser Add-on, and you can manage Google ad personalization at My Ad Center. Most browsers also let you block or delete cookies.
4. Text messages and calls
If you provide your phone number and consent on the form, you agree that Summit Health may contact you by phone, text message or email about your request. Message and data rates may apply. You can opt out of text messages at any time by replying STOP, or ask us to stop contacting you by calling the clinic. Your consent to be contacted is not a condition of receiving care.
5. How we share information
We do not sell your personal information. We share information only:
- With service providers who operate the Site and our scheduling, email, hosting, form-handling and analytics tools, under obligations to protect it
- With clinicians and staff involved in your care
- When required by law, subpoena, or to protect the safety or legal rights of patients, staff or the public
- In connection with a merger, acquisition or sale of clinic assets
6. Protected health information
Once you become a patient of Summit Health Clinic, information created or received in connection with your care is protected health information governed by HIPAA and by our Notice of Privacy Practices, which is provided at the clinic. That notice, not this policy, governs how we handle your medical record.
7. Data retention and security
We keep appointment request information only as long as needed for the purposes described above and to meet legal and recordkeeping requirements. We use reasonable administrative, technical and physical safeguards to protect information, though no method of transmission over the internet is completely secure.
8. Your choices
You may ask us to correct or delete the contact information you submitted through this Site, or ask us to stop contacting you, by emailing scott@summithealthmo.com or calling (417) 232-3000. Depending on where you live, you may have additional rights regarding your personal information, and we will honor those rights as required by applicable law. Requests about your medical record are handled through the clinic under HIPAA.
9. Children
This Site and the services described on it are intended for adults 18 and older. We do not knowingly collect personal information from children.
10. Changes to this policy
We may update this policy from time to time. When we do, we will revise the "Last updated" date above.
